Loss and Trauma Class Feedback Study

Informed Consent

      This research is being conducted by Michelle Mamberg, Ph.D., professor in the Department of Psychology at Hanover College.  The study in which you are asked to participate, entails filling out a demographic questionnaire and survey.   The survey contains questions regarding the "Loss and Trauma" class you took with Dr. Mamberg. There will be no limitation on your answers to these questions, you may write as much as you want.  The entire study will probably not take more than 30 minutes. There are no known risks involved in being in this study, beyond those of everyday life.  You may even find the experience of writing the answers informative in some way.  The information you provide during the study is completely anonymous; at no time will your name be associated with the responses you give. Some portions of your typed answers may be represented at a professional conference, however, any personally identifying information will be omitted.  At the end of this study, you will have the choice to keep your data out of the professional presentation, or you may give permission for it to be used at the conference.
